BECOMING INDEPENDENT JOB DESCRIPTION

POSITION: Community Coach

PROGRAMDay Programs

POSITION  DESCRIPTION: This position assists individuals in identifying and achieving personal goals in their lives through community inclusion, volunteerism, employment, education and other social/personal interests.

SUPERVISES: N/A

SALARY: $16.75/hour (with longevity increases)

STATUS; Non-exempt 

CRITICAL JOB FUNCTIONS

Possesses the professional skills and abilities specific to the technical aspects of the position

PROVIDE SUPPORTS 

  • Support and participate alongside individuals in vocational, volunteer, and other meaningful community activities that align with the organization’s philosophy.
  • Assist in establishing and maintaining social connections with peers in the community, with a focus on reducing relationships with paid supports. 
  • Support individuals with instruction as requested or needed to promote skill development and independence and in some situations, with personal care.

DEVELOP AND IMPLEMENT SERVICE PLANS

  • Participate in developing and implementing instructional and behavioral plans based on the person’s objectives.
  • Conduct interviews and assessments in preparation for service plan development.
  • Schedule, coordinate and participate in team planning meetings.
  • Conduct lessons and activities for individuals drawn from the Individual Service Plans/Person Centered Plans.

TRANSPORTATION OF PERSONS SERVED

  • Transport persons served to and from volunteer sites and community activities.  This will frequently be a personal vehicle but may include the operation of BI vehicles or riding public transportation.
  • Assist passengers as needed, encouraging good passenger etiquette and safety.
  • Maintain records and submit accurate reports as required by BI transportation and/or other related agencies.
  • Ensure inspection and cleanliness of the vehicle before/after each use.

KEEP RECORDS AND WRITE REPORTS

  • Maintain individual progress records; write and submit accurate reports as per procedures established.
  • Ensure correct data entry and accuracy of all time and attendance and work records. 

DEVELOP AND MAINTAIN EFFECTIVE RELATIONSHIPS

  • Develop and maintain effective relationships with persons served, peers, supervisors and other staff, affiliated agency staff, and members of the community.
  • Attend, actively engage, and participate in weekly staff meetings, training sessions, and other relevant meetings.

OTHER

  • Proof of COVID-19 vaccination or valid medical or religious exemption is required as a condition for employment. Individuals who are in the vaccination process ie; are only partially vaccinated/have only received the first dose in a two dose series or are awaiting the 14 day post vaccine waiting period for full vaccination status, must be fully vaccinated prior to beginning employment at BI. Individuals with a valid medical or religious exemption must undergo a weekly COVID-19 testing.
  • Provide support as a Direct Support Professional working in any BI program including but not limited to, providing in-home care, transportation and in-community services for persons served.
  • Executes position specific job duties and other duties as assigned at the discretion of BI.

QUALIFICATIONS & REQUIREMENTS

EDUCATION:  High school diploma required; College coursework in Special Education, Rehabilitation, or a closely related field is preferred.

EXPERIENCEPrevious experience working with individuals with intellectual/developmental disabilities is desirable. Good customer service skills and an interest in helping others pursue and achieve their best life is required. 

PHYSICAL REQUIREMENTS:

  • A physical condition that will permit frequent bending, turning, and lifting without injury.
  • Ability to walk and stand for prolonged periods of time.
  • Ability to assist individuals in wheelchairs.
  • Provide personal care to individuals including assistance in the restroom and feeding.
  • Lift at least 40 pounds.
  • Clearance of pre-employment Functional Capacity Evaluation with no restrictions that cannot be reasonably accommodated without undue hardship.

  • Valid California driver’s license
  • Insured, dependable vehicle

  • First Aid and CPR certification
